2012 Annual Pain Point Survey Terms and Conditions
Background
----------
The microISV Research Alliance is a network of academic research labs at U.S. universities. We are a non-profit organization. Our goal is to build a body of peer-reviewed, academic research surrounding the microISV community.
Survey
------
Participation in this survey is strictly voluntary. The only required survey responses are (1) you agree to the terms and conditions and (2) you acknowledge you have read the survey information letter (available at http://survey.microisvresearch.org/SurveyInfoSheet2012.pdf). In order to minimize invalid responses, a valid email address is required to gain access to the survey.
During the survey, a minimal amount of personal information is requested. All these questions are optional. You may voluntarily provide your microISV name, website URL, and company profile, which will qualify you for inclusion in a public listing of survey participants, with a live link to your site. This participants' page will be maintained for approximately one year, until the results of the next survey are published. If you choose not to provide this information, we will not list you as a participant.
Personally identifying information will not be published with other survey responses, such as selected pain points or demographics.
Limit one survey completion per individual.
Results

At the end of the survey period, the results of the survey will be compiled. All personally identifying information will be removed. Where such information cannot be removed, it will be aggregated to protect anonymity. The results will then by made available to all participants.
Future Communications
---------------------
We will contact each participants via email in the following instances:
* When a request to take the survey is made.
* When the survey period begins.
* When the survey period is ending, if the survey has not been completed.
* When the results are available for download.
* When the next survey is released.
* Within one year of completing the survey, we may contact you with an offer to take a second, different survey. If we do contact you, this will be a one-time event.
Drawing
-------
No purchase is necessary. Your chance of winning is determined by the number of survey participants who ask to be included in the drawing and the final number of prizes. For example, if one hundred microISV operators respond to the survey, everyone enters the drawing, and the final number of prizes is five, then your chances of winning are 1 in 20 (i.e. 5 out of 100). Respondents may win only a single prize and the prizes will be assigned randomly based on the order of the drawing.
The list of prizes can be found at http://survey.microisvresearch.org/drawing.php. As of the last modification to these terms, the list includes:
* From E-Junkie (http://www.e-junkie.com/): One year of free E-junkie service at our $10/month plan level, allowing up to 20 different products sharing up to 100 MB of download storage space.
To be included in the drawing, respondents must indicate so by checking the box labelled "Yes, include me in the prize drawing" and must provide a "Drawing alias" (to keep things anonymous). These options can be found on the sixth page of the survey. Of course, respondents may choose to provide their real name in the "Drawing alias" question, in which case you will not be anonymous and you may have your real name read aloud in a video to be posted publicly on the Internet.
The drawing will be held within one week of the close of the survey. For each prize, we'll throw everyone's name is a hat (who hasn't won a prize, yet), then draw out one name. That will be the winner of that prize. Since we're on a college campus, we'll probably grab the first random computer science student who wanders by to actually reach in and get a name (without looking, of course).
The winners will be contacted via email immediately following the drawing. If a winner does not respond within seven days, a second email will be sent. If, again, no response is received for seven days, we'll send a third and final email. After a third week of waiting, we'll announce a new drawing and the process will repeat. Of course, we'll throw out anyone who isn't checking his email. Eventually, we will give away these prizes!
The winners will not be publicly identified, unless they choose to be. We'd like to announce who won, but we respect your privacy.
And one more thing... Void where prohibited.
